When a suspect requests a DMV hearing – either at a DMV office or over the phone – he/she must be prepared to answer questions. Suspects should have the following information handy when requesting a DMV hearing:

• Name and driver’s license number
• The date of arrest or the date the notice of suspension was issued (date appears on the notice itself)
• Whether or not the suspect submitted to a chemical test
